In a study of 89 women with hyperandrogenism due to persistent adrenarche syndrome, finasteride produced a 93% reduction in facial hirsutism and a 73% reduction of bodily hirsutism after 2 years of treatment. Finasteride is most effective on the crown but can reduce hair loss in all areas of the scalp. In comparison a full head of hair usually has 120 hair follicles per square centimeter scalp. Two meta-analyses found finasteride's efficacy caused about 15% hair regrowth. It is usually taken orally but there are topical formulations for patients with hair loss, designed to minimize systemic exposure by acting specifically on hair follicles. Propecia is an oral drug that is taken daily to reduce the amount of testosterone converted to DHT, and it can reverse hair loss in some men.
Scientific evidence does not support this popular belief, as previous studies show no consistent correlation between baldness and high testosterone (National Library of Medicine). At its core, genetic factors determine how sensitive your hair follicles are to DHT. As DHT binds to androgen receptors in the hair follicles, it causes them to shrink and produce thinner, shorter hair gradually. Testosterone itself doesn’t directly cause baldness; instead, an enzyme converts testosterone into dihydrotestosterone (DHT), a more potent androgen (Cleveland Clinic).
Newer terminology developed to express the different patterns of presentation in men and women and the uncertain role, and frequent absence, of androgen excess in women. Merck's patent on finasteride for the treatment of BPH expired in June 2006. In 1992, finasteride (5 mg) was approved by the US Food and Drug Administration (FDA) for treatment of BPH, which Merck marketed under the brand name Proscar.

Biotin, or vitamin B7, may help promote hair, nail, and skin health due to its role in supporting keratin synthesis. A variety of laser treatments aim to strengthen and promote regrowth. It can be used to treat a receding hairline and bald spot up top (but not hair loss at the temples), according to the National Library of Medicine. But when you start TRT, you may gain—or lose—hair on your face, head, or body. If your receptors are highly sensitive, even small amounts of DHT can easily trigger them, leading to increased hair loss. When your T levels rise, your body metabolizes more T into DHT. During puberty, DHT is what caused you to sprout body and facial hair.
Estrogen therapy increases sex hormone binding globulin and, like aging men, this reduces the amount of free, active testosterone in the body. In fact, as men age, testosterone levels drop very gradually, about 1% to 2% each year — unlike the relatively rapid drop in estrogen that causes menopause. Women with high testosterone levels, due to either disease or drug use, may experience a decrease in breast size and deepening of the voice, in addition to many of the problems men may have. One treatment available for many of these problems is spironolactone, a special type of diuretic (water pill) that blocks the action of male sex hormones. Part of this may be due to the difficulty defining "normal" testosterone levels and "normal" behavior. When testosterone levels rise too high, the brain sends signals to the pituitary to reduce production.
Inflammation, scarring, or scaling of the scalp suggests a different diagnosis, as pattern hair loss is usually unaccompanied by these signs. Male pattern hair loss typically presents as a receding hairline and hair miniaturization on the frontal and vertex scalp. This review focuses on clinical presentation, diagnosis and treatment of pattern hair loss. Merck was awarded a separate patent for the use of finasteride to treat pattern hair loss and it expired in November 2013. Upon maturation, these individuals were observed to have smaller prostates which were underdeveloped, and were also observed to lack incidence of male pattern baldness. In 1942, James Hamilton observed that prepubertal castration prevents the later development of male pattern baldness in mature men. In parallel, circulating levels of testosterone increase by approximately 10%, while local concentrations of testosterone in the prostate gland increase by about 7-fold and local testosterone levels in hair follicles increase by around 27–53%.
As of 2008, there is little evidence to support the use of lasers to treat male-pattern hair loss. Although it is generally accepted that male pattern baldness follows a pattern of autosomal dominant inheritance, more recent research has shown that approximately 80% of bald men have bald fathers. It appears to be the result of genetic changes that make the activity of hair follicles on the scalp become sensitive to the presence of androgenic hormones, cholesterol, and proteins such as insulin-like growth factor.citation needed Male-pattern hair loss begins above the temples and at the vertex (calvaria) of the scalp.
Male pattern hair loss and female pattern hair loss are now the favored terms. A small retrospective study reported that finasteride was effective in the treatment of acne in women with normal testosterone levels. Preliminary research suggests that topical finasteride may be effective in the treatment of pattern hair loss.
